How it works in CareNote

Setting up PCO Form automation
Connect Planning Center
Make sure your PCO account is connected in Team Settings
Navigate to PCO Forms
Go to Admin → PCO Forms. Your forms sync automatically from Planning Center.
Configure automation rules
Select a form and set up automation: choose the category, default assignee or group, and field mappings.
Form submissions create care requests
When someone submits the form in PCO, a care request is automatically created in CareNote
Team gets notified
Assignees receive email/SMS notifications about the new care request
