“Track hospital and care visits for our staff and team”

— What churches tell us they need

How it works in CareNote

CareNote makes it easy to log hospital visits, nursing home check-ins, and any other pastoral care touchpoint. Each visit is tied to the person's care history so your whole team can see what's been done and what's next.

Step-by-step guide

1

Navigate to Interactions

From your Dashboard, click “Create Interaction” or go to a person's profile and click “Log Interaction”

2

Select the interaction type

Choose “Hospital Visit” from the interaction type dropdown

3

Select the person

Choose the profile of the person you visited from your congregant list

4

Set the location

Select or create the hospital/facility as a Place. This helps track visits by location.

5

Add visit details

Record the date, time, duration, and notes about the visit. You can also set the sentiment (positive, neutral, concerning).

6

Set follow-up if needed

Check “Requires follow-up” and set a date. CareNote will remind you when it's time to visit again.

What you get

  • Complete visit history for each person
  • Track visits by hospital/facility location
  • Automatic follow-up reminders
  • Team visibility—everyone sees the care that's been provided
  • Reports showing hospital visit activity
